Patient Grievance or Complaint Form

Have a concern about your care?

Patients have the right to file a grievance regarding the care or treatment they received (or believe they should have received), or to file a complaint about the Tuolumne Me-Wuk Indian Heath Center or its staff without fear of discrimination or retaliation. All grievances and complaints will be handled in a fair, efficient, and timely manner. All submissions are treated confidentially and will be given serious consideration.

To submit a patient grievance or complaint, please complete the form clicking the button below. Once you select “Submit,” your form will be sent directly to the Compliance Department for review.

Paper forms are available at all clinic locations. Once completed, please submit the form to the Compliance Department by:
