Continuity of Care and Medication Management

Working together for Safe and Effective Treatment

Our goal is to help you manage your health safely and effectively through responsible use of medications and consistent care.

Managing medications requires teamwork between you, your medical provider, and your care team.

Our Commitment to You

CONTINUITY OF CARE AND MEDICATION MANAGEMENTProvide consistent care by coordinating your prescriptions and medication plans through one primary provider or care team.

  • Review your health needs and determine which medications are necessary and safe for you.
  • Prescribe responsibly, following evidence based practices and federal/state regulations.
  • Monitor your progress and make adjustments to your medications as your health needs change.
  • Communicate clearly about how your medications work, possible side effects, and what alternatives may be available.
  • Coordinate care with specialists,  pharmacies, and other members of your care team to ensure all medications work well together.

YOUR ROLE IN SAFE MEDICATION MANAGEMENT

To help us care for you safely, please:

  • See your assigned provider for medication management and refills whenever possible to maintain continuity of care.
  • Take medications only as prescribed — do not share, stop, or change doses without talking to your provider.
  • Bring an up-to-date medication list (including over-the-counter and herbal products) to each visit.
  • Inform your provider about any side effects, concerns, or changes in your condition.
  • Avoid requesting specific medications by name — your provider will determine what is clinically appropriate for your situation.
  • Participate actively in your care plan, which may include lifestyle changes, counseling, or non-medication options.

Why This Matters

  • Continuity of care helps prevent medication errors, unsafe combinations, and misuse.
  • Responsible prescribing protects you, your family, and our community.
  • Working together ensures your care is safe, effective, and culturally respectful.

Together, we build trust through continuity and care.
